A typical private GP day in Malaysia is not “all cash” or “all insurance”. The same morning mixes walk-in cash, corporate panels, and TPA (third-party administrator) visits. A clinic management system has to keep those invoices honest: the patient still walked in, but who owes which ringgit is different.
This is an operator note for the visit itself. For module shortlisting, use the Malaysia buyer's guide. For LHDN submission patterns (B2C vs B2B), use the MyInvois primer and the cash GP example.
Mental model
“This patient belongs to ABC panel and ABC pays (part of) the bill.”
That is not the same as “the invoice is unpaid”. In Desk Clinic:
- A panel organisation is the company, insurer, or TPA you bill.
- A patient panel membership records that this patient is on that panel (with history—membership edits do not rewrite old invoices).
- The invoice snapshot stores patient responsibility and panel responsibility in sen. That snapshot is the source of truth for that bill.
- Patient collectible is what you still need from the patient at the counter.
- Panel receivable is what you still expect from the panel. It is not a fake
Paymentrow.
If two memberships are active, staff must Select (panels + self-pay). The system must not guess.
Run this in every demo
Use one real patient who has (or can be given) a panel membership.
- Book or walk in the visit and open the encounter as usual.
- Create the invoice from that visit—not a spreadsheet export.
- Allocate responsibility: consultation copay on the patient, the rest on the panel (or whatever your agreement says).
- Confirm the counter sees patient collectible, not the full total, when the panel share is real.
- Confirm the receivables list can show awaiting panel settlement instead of labelling the patient “unpaid” for money they do not owe today.
- Record a cash/DuitNow/card payment only for the patient share. Do not insert a payment for the panel share until money actually arrives.
Desk Clinic’s panel engine can propose coverage from dated agreements (full panel, patient copay, panel up to a limit). It is not a claims portal: you still submit to the TPA the way you do today. Later phases for e-claim APIs are out of scope for this article.
What “broken” looks like
- One invoice total with no patient vs panel split
- Changing today’s membership silently rewriting last month’s bills
- A “panel payment” created just to make the invoice look paid
- Auto-selecting a panel when the patient has two cards in the wallet
- Staff told to track panel balances in Excel beside the CMS
If a vendor cannot show the split on a live invoice, panel billing is a slide, not a product.

FAQ
Does Desk Clinic submit TPA claims for me?
No. Desk Clinic keeps visit-linked invoices, memberships, and responsibility snapshots so cash, corporate, and TPA-style billing stay on the same patient record. Claim portals and insurer file formats still belong to each panel—validate your top panels in a trial.
What if the panel pays later?
Keep the invoice issued. Panel receivable stays open until settlement. Do not create a payment until funds are received, the same rule as DuitNow confirmation for patient collection.
How is this different from insurance claim modules in hospital HIS?
Panel/TPA here is outpatient clinic billing: who is responsible on this visit’s invoice. It is not inpatient coding, not a full e-claim switch, and not an employer benefits portal.
